The role is part-time salaried to start in May 2020 – but we will be flexible for the right person and there is the option of partnership available.
We are a semi rural – approx 5,300 patients – dispensing partnership who believe in continuity of care. We are a high earning, good QOF scoring practice fully involved in Clinical Commissioning Group and Primary Care Network roles.
We are 10 minutes from St Austell and 20 minutes from Truro, working out of a purpose-built premises with no branch surgeries.

DIABETES & RESPIRATORY NURSE

The role involves:
Managing Diabetes care and its complications;
Managing COPD care and its complications;
Act as an advisory role to patients and doctors;
To prevent avoidable admissions to hospital;
Reduce referrals into the traditional diabetes/respiratory outpatient services;
We already achieve the highest targets for QOF which you will be expected to continue.
